"I was sleeping when my boyfriend hurriedly came to our bedroom and asked to see my phone. When I asked him why he needed to see my phone, he forcefully took it from me and then pressed it. Thereafter he started to assault me all over my body with his open hands and he also kicked me," reads the affidavit.
The affidavit further reveals that she sustained major injuries on her face as a result of the incident.
Police records indicate that the case was also withdrawn the same day it was reported but then later reinstated.
However, after appearing in court, the records seen by Sunday World shows that the docket was mysteriously closed again.
Mukwevho, 31, told Sunday World that this was his last month at the Joburg music radio station and begged for the story not to be published stating that it was going to destroy his career. "I apologise for what I did, I'm really sorry. I won't do it again. I will never be hired again if this comes out," he said.
Midrand police spokeswoman Matome Tlamela confirmed that Mukwevho was a VIP guest for a night at the station's police cells.
"The suspect was arrested and appeared at the Midrand Magistrate's Court ," she said.
Tlamela also confirmed that the docket had since been closed after the complainant withdrew the charges against her lover.
While at YFM, Mukwevho was suspended and later fired by the youth station in 2010 after a complaint was registered with the Broadcasting Complaints Commission of South Africa.
The complaint stemmed from the presenter's mocking of gays, including his then-colleague and producer Junior Dikwa.
He had also played an audio clip of some Ugandan pastor who made fun of gay people.
Mukwevho later apologised for his remarks but was still dismissed from the station.

Highveld Stereo presenter and club DJ MacGyver Mukwevho survived possible jail time by a whisker after his girlfriend and baby mama withdrew her assault charges this week.
The woman, who cannot be identified for legal reasons, had opened a case against the former YFM DJ at the Midrand police station two weeks ago, claiming he had kicked and beat her to a pulp.
She withdrew the charges of assault with intent to do grievous bodily harm against him on Wednesday.
The young radio maestro, who left YFM under a cloud after making derogatory statements against gay people, spent a night in the police holding cells as a result of the charges.
She told the police that the incident, which happened at the couple's Midrand townhouse, was sparked by an argument over her phone.
He had accused her of cheating on him after seeing her smiling several times while on her phone, her statement read.
