Eight people died in a head-on collision between a truck and a minibus taxi in Middelburg, the Eastern Cape health department said on Sunday.
The accident took place 46km outside Graaff-Reinet around 21:00 on Saturday, said spokesperson Sizwe Kupelo.
The taxi had been travelling from Uitenhage to Gauteng.
Kupelo said the truck driver and seven taxi commuters died. Four critically injured people were referred to Livingstone Hospital in Port Elizabeth.
Six others were discharged from hospital.
Premier Phumulo Masualle offered condolences to the families of the deceased and wished those in a hospital a speedy recovery.
“The Eastern Cape provincial government is urging all motorists on the road to exercise extra caution and avoid unnecessary loss of life,” said Kupelo.
