Smoke from the nearby veld was blamed for the 17-vehicle pile-up that left 15 people injured and four in a serious condition on the N2 North-bound near Stanger.
It is believed that the wreckage of several cars, which had collided earlier, was smashed into by oncoming traffic as the smoke obscured the freeway.
Netcare 911 provincial spokesperson Jeffrey Wicks said: "Paramedics arrived on the scene to find wrecked trucks and vehicles littering the scene, with the injured moving to the roadside after climbing from their cars."
Wicks said all the injured were treated and stabilised on the scene before being taken to area hospitals.
He appealed to drivers to gradually reduce speed and use hazard warning lights whenever travelling through smoke-affected areas.
In another accident, schoolchildren aged from 12 to 16 were seriously injured when their taxi was struck by a bakkie on the Mount Batten Drive near New Germany junction.
It is alleged that the taxi driver attempted a right turn into New Germany Road but as he steered his vehicle a bakkie was unable to stop in time and ploughed into the side of the taxi, pushing it sideways and causing it to mount the curb.
The bakkie came to rest a few metres away from the point of impact, leaving two pupils seriously injured, while six sustained minor injuries.
Wicks said the Advance Life Support Paramedics treated all the injured on the scene before being taken to a nearby hospital.
Meanwhile, a man is fighting for his life and a woman has been seriously injured after their vehicles collided head-on on the M4 Ruth First Highway near Westbrook, Tongaat.
