Paramedics who attended to the victims at the accident scene found the five, including a six-year-old, trapped inside a wrecked Ford Escort.
The car they were travelling in collided head-on with a truck at about 11am yesterday.
Netcare 911 spokesperson Chris Botha said: "Tragically they all died on impact because of the extensive injuries they had sustained.
"There were two survivors who were critically injured."
Botha said the driver of the delivery truck and his assistant were both seriously injured.
"The were stabilised at the scene and taken to a hospital for further treatment."
Meanwhile, the owner and the driver of a bus that crashed, killing 28 people in Western Cape, were remanded in custody yesterday.
Captain Frederick van Wyk said bus owner Malinga Nonoma and his brother Siza Nonoma appeared briefly in the De Doorns periodical court on Wednesday.
He said their case was postponed for further investigation and they would appear in court again on Monday.
Both were charged with 23 counts of murder.
"They did not apply for bail and were not asked to plead," Van Wyk said.
The accident occurred earlier this month.
Nonoma allegedly lost control of the bus causing it to overturn on the N1 between De Doorns and Touwsrivier. Among the dead were four children.
The bus, which was certified to carry 64 people, had 77 passengers. It was on its way to Cape Town.
The SABC earlier reported that Eastern Cape Premier Noxolo Kiviet and members of her executive council were expected to attend a memorial service for the bus victims at Ngcobo in the Transkei.
The victims would be buried separately at Ngcobo, Ngqamakhwe and Cofimvaba over the weekend.
It was reported that the government would reimburse the families who had paid for transportation to and from Western Cape.
