This came out during a formal inquiry in Pietermaritzburg by the Department of Labour into the massive explosion of a furnace at the plant on February 24.
Reading a statement prepared by Assmang's former operations manager, Robert Burger, Willem le Roux said the design and construction of the boiler was safe.
Burger died in a car accident in November last year.
Burger said the furnace was commissioned in 1990 and that the company had spent more than R30million refurbishing it.
An overhaul of the furnace had taken place in 1995 and that was followed by another in 1999.
Another overhaul had taken place in 2002 and the last one was done in 2005 at a cost of R18,5million.
