Police spokesman Inspector Zilungile Nkekamba said the accident occurred between Tarkastad and Cradock on Sunday.
A small vehicle and a taxi collided head on.
All three occupants of the small vehicle were killed as were 12 people in the taxi. Four others in the taxi were seriously injured.
