Vusi Mathebula, 30, of Bethal shot his girlfriend Nomadlozi Mthimunye, 25, after she apparently broke off their relationship.
He then gunned down her brother Paulos, 21, before shooting himself at their home in Dundonald near Ermelo.
Mathebula had earlier attempted to shoot a local induna, Samuel Vilakazi, who managed to escape unhurt and reported the matter to the police.
