"Two cars collided head-on on the R617 Boston-Bulwer road in the early afternoon," said Netcare 911 spokesman Jeff Wicks.
The accident happened when one vehicle veered into the oncoming lane and collided with another car.
Four were killed, one was critically injured and four others were in a serious condition.
The injured were taken to the Northdale and Edendale hospitals in Pietermaritzburg, Wicks said.
lA motor cyclist was seriously injured when he collided with another rider on the N2 near Durban on Saturday morning.
Netcare 911 spokesman Jeff Wicks said the man fell off his motorcycle after clipping another biker while changing lanes.
