The Mpumalanga department of health and social services is investigating claims that a woman died in a hospital as a result of "the province's failing health system".
The Sedibe family of Phola Trust, Hazyview, has blamed "the failing health system" after Lindiwe Sedibe died at Themba Hospital.
Lindiwe Sedibe's daughter, Flora Makutu, said her mother was admitted to Themba Hospital on March 28 to have her jaws checked.
"The doctors found that she had meningitis," Makutu said.
"She was given antibiotics, to which she reacted. The treatment was changed within two days."
Makutu said Sedibe's lungs gave in and she died a week after starting treatment for meningitis.
The spokesman for the health department, Mpho Gabashane, said the matter was receiving attention.
