She was honoured for protecting consumers against unfair business practice, abuse and danger, and helping to improve their lives.
The awards were managed by Two-Tone, who were meant to pay the winner once the DTI paid the money to the company. The DTI had paid the company.
Some winners said that Two-Tone paid them only after they threatened to expose it in the media.
In July, Two-Tone acknowledged receipt of payment and undertook to pay the Madlala's, but they have not done so.
The department found it disturbing that Two-Tone had not honoured the payment to Madlala's estate.
The department has undertaken to investigate the matter.
Two-Tone has not responded to Consumer Line's queries about the allegations since last Friday.
